Policies and Procedures
Who do you treat:
At this time we are only able to see patients aged 18-64 who are currently located in either Virginia or Indiana.
Billing and Payment Policies:
Payment is expected at the time the service is provided and will automatically be billed to the credit card on file after your appointment.
Billing for appointments:
Payment for appointments will be made by credit card. All patients must have a credit card on file to schedule, but please inform us if alternative payment methods are needed. The following are the rates for scheduled appointments:
New appointment:
Comprehensive Psychiatric Assessment, 60-75 minutes, $450
Follow-up appointments:
Focused Follow-Up Session, 25 minutes, $225
Comprehensive Follow-Up Session, 50 minutes, $375
Specific lab testing is often recommended during initial evaluation and may be ordered as clinically indicated based on your symptoms and goals. This may be billed to your insurance or paid out of pocket depending on the lab you select and your insurance coverage.
Cancellations:
Cancellations with less than 48 hours notice and missed appointments are billed at the rate of the scheduled appointment.
Fees for services between appointments:
Clinically-related services provided between scheduled appointments are billed at a prorated hourly rate, although for brief or limited services (less than 15 minutes), these will generally be waived.
Note: Non-clinical phone, email, or text contact (such as scheduled or billing questions are not subject to fees).
Phone calls with family members or other providers such as therapists or primary care doctors:
Clinical questions answered over email or phone will be billed based on the time it takes to respond to the question.
Insurance Coverage:
I am not in-network for any insurance panels, and am considered an "out of network provider" for commercial insurance plans. While I do not contract with insurance companies, I do assist my patients by providing the paperwork necessary to submit into their insurance carriers for reimbursement (a "superbill"). I am not able to negotiate or submit claims with insurance companies. Please be aware that your insurance provider may not reimburse you for any, or may only reimburse you for a part of, the charges for my services. Insurance companies generally do not reimburse for administrative services or missed appointments.
Medicare/Medicaid:
I am not able to see any patients with Medicare and/or Medicaid at this time, even if willing to pay out of pocket.
Limits of Service:
I do not provide emergency and/or crisis services, disability evaluations, worker's compensation evaluations, forensic evaluations, or provide legal services or testimony. Should you require legal testimony at some point during our treatment together, you will need to retain an independent forensic psychiatrist. This is to maintain the therapeutic relationship developed in treatment.
Fees:
Fees are subject to increase on an annual basis. You are entitled to a Good Faith Estimate upon request and will be provided one annually at a minimum. For more information, please go to https://www.cms.gov/nosurprises. You will also be notified in advance of any fee increases.
